Revisión sistemática sobre el control neuronal para restaurar la función motora tras paraplejía

Resumen

Las interfaces cerebro-computadora (BCI, por sus siglas en inglés) son una estrategia para restaurar funciones motoras y sensoriales en personas con lesión medular, superando las limitaciones de los tratamientos convencionales. Esta revisión sistemática, conducida con la guía Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), analizó doce estudios publicados entre 2020 y 2025, incluyendo casos clínicos, ensayos aleatorizados y multicéntricos. La búsqueda se efectuó en PubMed y Google Scholar, complementada con Rayyan para eliminar los duplicados. Se incluyeron ensayos clínicos o experimentales en humanos sobre paraplejía con intervenciones de puentes neurales (implantes corticales, estimulación epidural, bypass neural, interfaces cerebro-médula) y reportes de resultados motores o sensitivos; se excluyeron estudios en animales, otras patologías neurológicas, publicaciones previas a 2020, documentos sin revisión por pares y artículos sin traducción accesible en inglés o español. Los resultados muestran avances en la restauración de la función manual, la integración funcional y la autonomía en entornos domiciliarios; sin embargo, la recuperación de la marcha es un objetivo menos explorado. Predominaron los sistemas invasivos con alta precisión en la decodificación neuronal frente a alternativas no invasivas que priorizan seguridad y facilidad de uso. Se reportaron experiencias positivas con plataformas híbridas que combinan BCI con estimulación eléctrica funcional o exoesqueletos. Las principales limitaciones incluyen tamaño muestral reducido, heterogeneidad y escasez de seguimiento a largo plazo. Se concluyó que las BCI cambian la neurorrehabilitación de personas con paraplejía, con alto potencial clínico aún en consolidación, dependiente de estudios multicéntricos más amplios y protocolos estandarizados.
